D. Overall Purpose Of The Job (Brief description of the primary purpose of this position)

• As Occupational Health Nurse (OHN), the nurse is primarily accountable to coordinate all Occupational Health related activities on site as guided by Occupational Health Doctor (OHD).

• As Medic – Onshore, the nurse plays an integral role to support other medics at site in any emergency health care services to the workforce working at client site, oversees all aspects of clinical and medical management within the clinic, including medical evacuation support.

Patient Care

• Undertake clinical consultations and provide the requisite standard of service and care to patients attending the clinic.

• Develop and ensure continuity of patient care and appropriate follow-up and referral.

• Assist with the implementation of preventative medical care strategies, including structuring and performing medical screening services.

• Manage the delivery of competent and professional health services according to International SOS standards.

• Assist with ensuring appropriate patient triage and flow within the clinic.

• Coordinate with the International SOS Assistance Centres (AC) and Coordinating Doctors the procedures required and preparation of patients to facilitate evacuation.

Quality Assurance and Administration

• Work with the Chief Medical Advisor to optimise quality assurance and ensure that appropriate medical outcomes are consistently attained with respect to the provision of clinic services.

• Implement and follow at any time the Medical Services escalation procedure currently operating while managing medical cases at site.

• Escalate to / inform and seek advice from relevant AC regarding any prescriptive medications being made on site.

• Ensure patient's medical files and documentation are maintained to the requisite standards and in accordance with International SOS standards for clinic services.

• Assist in the maintenance, selection and control of the medical equipment and supplies available in the clinic when requested or required.

• Ensure an effective and organized delivery of medical services through a self-audit program.

• Track and report data on clinic utilization by using the standard in the approved format.

• Periodically self-audit the clinic standards and SOPP by using the self-audit tool and accordingly identify gaps if any and develop a plan of action with the support of the Assistant Operations Manager in order to keep the clinic at the highest score possible and ready for the official clinic audit.

Promotion of International SOS Services

• Ensure International SOS is represented in a professional manner.

• Ensure a Continuous Quality Improvement culture and system is maintained throughout the clinic.

• Assist in the promotion of International SOS' medical services with the client.

• Participate in International SOS' environmental, personal protection, disease and/or health promotions.

Occupational Health, Safety & Environment (OHS&E)

• Comply to Companys' OHS&E policy, standards and guidelines at all times.

• Promote good OHS&E practice at work place.

• Participate in OHS&E related activities from time to time.

• Contribute towards achievement of Companys' OHS&E objectives and key performance indicators

• Responsible and accountable to all aspects of safety and health at work place

OH&S Responsibility

• Participate in the development and maintenance of a safe and healthy workplace.

Medical Related

• Attending to all cases of medical ailments, trauma.

• Updating necessary paperwork eg Patient Contact Report (PCR), Medical Activity Log (MAL), etc

• Communicating for cases when required with Assistance centre and related paperwork eg. NOC/PCR, ecgs, photos.

Communication - Informing client representative /Supervisor of:

• All cases of trauma and work related injury.

• Communication with AC, per International SOS escalation procedures.

• Advise and arrangements for Emergency Disembarkation.

Fitness to Work (FTW)

• To implement FTW as advised by COMPANY procedure

• Identify all workers requiring FTW and maintain an up to date list

• Screen, verify and maintain fitness to work medical record including pre-employment, periodic, pre-placement and medical surveillance record

• Track FTW compliance in accordance with the COMPANY's FTW guideline.

• Provide FTW awareness.

• Arrange for return to work assessment, post illness, injuries as advised by Occupational Heralth Doctor

Public Health Services

Provide public health services e.g. Worksite Hygiene Inspection, control of communicable diseases including vector control supervision, food handling and catering assurance. These include the followings:

• Inspect Cafeteria

• Conduct worksite visits / walkabouts

Health Records

Maintain and submit identified health records as below;

• Outpatient records

• Monthly Health performance records (e.g. TROIF, Monthly Health Return,)

• Health surveillance records (e.g. audiometric test results, biological monitoring result)

Health Promotion

Participate and promote healthy work culture and lifestyle through health promotion activities and initiatives including:

• Conduct Health Promotion programs (e.g. health talks, blood donation, games, exhibitions, BMI Campaign, etc)

• Conduct Health Talk at location

• Conduct Occupational Health Awareness talks monthly (e.g. Hearing Conservation Program/Health Surveillance Stress, Repetitive Strain Injury, etc)

• Participate in HSE assurance activities

Medical Surveillance

• Identify and maintain a list of all workers requiring medical surveillance as per HRA recommendation.

• Track medical surveillance compliance (update list).

• Provide medical surveillance training

• Collect and collate COMPANY's medical surveillance data and submit to the relevant parties. The data includes, but not limited to:

o Clinic data

o Health performance records (e.g. TROIF, Monthly Health Return)

o Medical surveillance records (e.g. audiometric test results, biological monitoring result, etc.)

• Weekly check on first aid/pathogen stations.

• Weekly check on emergency immobilization devices and stretchers.

International SOS Medical Services operate in countries where international standards of medical care are not available or where cultural or language barriers make it difficult to receive appropriate health care. The Medical Service (MS) arm provides site specific medical services to global clients as determined by individual client contracts

Effective and consistent implementation of International SOS Policies, Standards, Procedures and Processes are important requirements of Clinical Governance that ensure client satisfaction, patient safety and effective risk management as well as continuous quality improvement (CQI) of clinical practice. Compliance with these requirements is monitored as part of the International SOS Integrated Management System that includes Clinical Governance Committees and meetings, internal and external audits, Site and Medical Service Centre reporting, clinical incident management and patient outcome monitoring, emergency drills, competency assessment etc.

· Maintain professional registration and qualifications in line with the requirements of the role.

· Ensure the Clinical Governance program for the Medsite is fully implemented and compliance with all Medsite procedures is maintained. This includes completion of:

o Clinical Governance Audits

o Medical Equipment Checklists

o Biomedical Equipment Maintenance is current

o Audit Plan of Action is developed

o Emergency drills are scheduled and occur.

o Pharmacy and Cold Chain Checklists

o Monthly Inventory and Stock Reports.

o Combined Checklists

o Site Monthly Reporting

· Provide clinical care and services within own scope of practice ensuring complete and comprehensive records are maintained that ensure continuity of care and case management as well as provide an auditable record of care that is compliant with the CPG's and Escalation Protocols.

· Actively participate in review of clinical outcomes, case studies, CG performance for the site through attendance (remotely) at meetings and other feedback forums.

· Ensure all required cases and potential issues are escalated in accordance with the Escalation procedure for Medical Services, Escalation protocols for Drs, Nurses and Medics and Incident Reporting Procedure.

D. Overall Purpose Of The Job (Brief description of the primary purpose of this position)

· To perform the role of Occupational Health Doctor, by providing emergency and primary health care and co-ordinating the relevant clinical and administrative tasks in keeping with the standard operating policies and procedures of Intl.SOS.

E. Key Responsibilities

He/she will be required:

1. To provide primary medical care on site to the client and agreed other parties.

2. To provide appropriate emergency medical/trauma response and care. Including the preparation and management of emergency evacuations of client personnel as directed by Intl.SOS Coordinating Doctor. In particular the Intl.SOS Coordinating Doctor shall be consulted in the event of serious or potentially serious medical or trauma conditions.

3. To provide appropriate medical and trauma care to client personnel at the Remote Site facilities. This is to be done in consultation and liaison with the Intl.SOS Coordinating Doctor as appropriate.

4. Assessment and immediate treatment of serious illness or injury including assessment, treatment, and regular follow up of all cases until there is complete resolution of the condition, the case is disembarked from the installation or when required, till a critically ill patient is delivered to receiving care in a suitable hospital. This care includes, but is not limited to:

Assessment and immediate treatment using advanced life support or Basic trauma life support skills in the event of serious trauma, shock, or cardiac arrest, including:

· Airway Management: including oxygen therapy, endotracheal incubation.

· Use of Monitor, defibrillator and ventilator

· Use of extrication and spinal immobilization devices

· Medication therapy. Intravenous therapy: with peripheral venous line placement and the administering of colloid/crystalloid solutions (to counteract hypovolaemic states due to bleeding, burns, severe diarrhea or vomiting

· Management of fractures and soft tissue injuries, including wound care management and proper local anaesthetic techniques, cleansing, debridement, suturing and bandaging.

Assessment and treatment of common illnesses. This care includes, but is not limited to:

· Diagnosis and management of the skin, eyes, ears, nose, throat, respiratory, gastrointestinal, cardiovascular and genitor-urinary systems.

· Ensuring appropriate referral cases which have not resolved as expected

· Local environmental health risks; be aware, recognize, prevent and treat as appropriate.

· Health Promotion; to integrate preventive services into the normal course of medical support duties

e.g. health promotion programs, immunization updates.

· Presenting information to employees on healthy lifestyles (alcohol, drugs, smoking, diet, weight control, exercise etc.)

· Counselling within a doctor's skill level. Information for all community members on preventive measures to be taken against risks specific to the area

1. To follow company procedures and guidelines as defined in Intl.SOS Standard Operating Procedures (SOPs) and Medical Emergency Response Plan (MERP).

2. To treat all medical information and information about client's activities obtained in the course of working on the project in the strictest confidential. In relation to medical information attention must be paid to Intl.SOS's Professional Code of Conduct.

3. To perform basic tests for any site agreed Occupational Health Program.

4. To direct the day-to-day activities of the medical facilities as the site representative for Intl.SOS.

5. To work proactively to ensure continuity in the delivery of the medical program to the client.

6. Provide Basic First Aid Training (noncertified) to all site employees using Intl.SOS material and client approved guidelines

7. Undertake periodic health and hygiene inspections of site facilities and provide assistance in identifying health and hygiene needs.

8. To provide supportive medical services on an emergency basis to the local community, as directed by the client's medical services representative.

9. Any other duties and responsibilities as may be required and agreed between Client and the Medical Services centre to further the Project Safety, Health, Hygiene and Environmental programs.

10. To travel to locations where emergency medical services are required in as timely a manner as possible. This will normally be directed by Intl.SOS Assistance Center or Medical Services centre in conjunction with the senior client supervisor or their nominated site representative. This travel may also include travel from site to nearest approved medical support facilities or handover to next medical evacuation team as directed by the Intl.SOS Coordinating Doctor

11. Stabilization and preparation of patients in cases of emergency medical evacuation.

12. Provide support to manage clinic activity, epidemiology, drugs, and stocks.

13. Maintain medical supplies and equipment as per Intl.SOS standards

14. Inspect and maintain First Aid and emergency eye wash stations if required.

15. Assist in safety inductions for all new arrivals to site.

16. To work alongside Safety team to participate in accident/incident investigations.

17. Assist in basic level First Aid training for rescue or emergency response teams.

18. When not involved with patients, assisting in non-medical duties if required: Filing, T-cards, maintaining POB. Arrange manifests for personnel/freight on helicopter and other duties associated with personnel movement and relevant to the site requirements.

19. When necessary, assist radio operator with assignment of rooms and emergency stations and muster points. (For offshore facilities)

20. To strive for personal development and achievement according to the MS career ladder program.

21. Assist Training Manager in teaching resource development and preceptor programs.

22. Ensure that Intl.SOS is represented in a professional manner and that this is respected in terms of conduct, appearance and service.

23. To prepare the agreed upon reports describing the activities of the medical facilities and to ensure that all necessary reports are provided on any schedule agreed between Intl.SOS and client.

24. Maintain accurate and complete medical records including responsibility in maintaining confidentiality of medical records.

· Compile monthly statistics on medical treatment provided. Submit operations reports to site management and Intl SOS, as required

OH&S Responsibility

· Participate in the development and maintenance of a safe and healthy workplace.

· Undertake all mandatory OH&S training as required.

· Comply with any reasonable instructions, policies, procedures or safe work practices given by Intl.SOS in adhering to safe work procedures.

· Co-operate with management in its fulfillment of its legislative obligations.

About You
Skills and Qualifications

• Medical Degree (MBBS or equivalent) from a recognized institution and post graduate qualification in Occupational Health / Occupational Medicine. Full Registration with the Malaysian Medical Council.
• Possess at minimum, a valid Occupational Health Doctor (OHD) certification from NIOSH Malaysia. Other post graduate Occupational Health/Occupational Medicines maybe considered. Preferably 5 years' experience or more practicing Occupational Health post attainment of postgraduate qualification

Shall have at least five (5) years of working experience post-graduation with at least two (2) year of working experience in Occupational Health or as an Offshore Medic. 5 2 Shall be a certified Medical Assistant/Assistant Medical Officer/Nurse and registered with the Board of Medical Assistant/Nursing, Ministry of Health Malaysia

a)Shall drive and implement the Risk Based Health Screening (RBHS) Programme prior mobilise CONTRACTOR and its SUBCONTRACTOR's staff. To Submit monthly reporting on outcome of the analysis or screening results to client

b)Assist to Maintain Health performance records (e.g., TROIF, TSA, Monthly Health Return)

c)Assist to maintain Health surveillance records (e.g., audiometric test results, biological monitoring result, industrial hygiene records, etc.)

d)Public health activities related records (e.g., potable water quality assurance and food handling and catering assurance)

e)Assist Shell /Shell Health in Incident Classification, Investigation and Reporting

f)Assist Shell /Shell Health in OH/IH related Incident investigation.

g)Assist HSE /incident owner in reporting and track closure of incidents.

h)Provide and participate in the medical emergency management as per respective Shell/Shell Health requirement. .

Job Summary

• Provides essential nursing services in occupational health services, including routine health assessment, medical surveillance, health promotion programs and occupational hygiene initiative.

Job Responsibilities

• Managing daily in-house clinical/ occupational health activities which include fitness to work, return to work, high risk group monitoring and etc.

D. Overall Purpose Of The Job (Brief description of the primary purpose of this position)

• As Occupational Health Nurse (OHN), the nurse is primarily accountable to coordinate all Occupational Health related activities on site as guided by Occupational Health Doctor (OHD).

• As Medic – Onshore, the nurse plays an integral role to support other medics at site in any emergency health care services to the workforce working at client site, oversees all aspects of clinical and medical management within the clinic, including medical evacuation support.

Patient Care

• Undertake clinical consultations and provide the requisite standard of service and care to patients attending the clinic.

• Develop and ensure continuity of patient care and appropriate follow-up and referral.

• Assist with the implementation of preventative medical care strategies, including structuring and performing medical screening services.

• Manage the delivery of competent and professional health services according to International SOS standards.

• Assist with ensuring appropriate patient triage and flow within the clinic.

Quality Assurance and Administration

• Escalate to / inform and seek advice from relevant AC regarding any prescriptive medications being made on site.

• Assist in the maintenance, selection and control of the medical equipment and supplies available in the clinic when requested or required.

• Ensure an effective and organized delivery of medical services through a self-audit program.

• Track and report data on clinic utilization by using the standard in the approved format.

• Periodically self-audit the clinic standards and SOPP by using the self-audit tool and accordingly identify gaps if any and develop a plan of action with the support of the Assistant Operations Manager in order to keep the clinic at the highest score possible and ready for the official clinic audit.
