Requirements

  • Has a minimum of five (5) years of clinical working experience as a registered nurse

We are looking for Hemodialysis Nurse (Post Basic/Non-Post Basic) for our Clinical department in multiple locations: Subang Jaya, Kota Damansara & Sri Petaling)

  • 6 days (Monday to Saturday including Public Holidays) rotating shift wor


Responsibilities:

  • For Head Nurse: must be able to lead a team of nurses
  • Wash, maintain and store all dialysis equipment safely.
  • Ensure that all supplies, equipment are available for use at all times.
  • Maintain a clean and safe environment within the dialysis centre.
  • Participate as a member of the team in carrying out haemodialysis procedures.
  • Able to perform needling of A-V fistulas, A-V graft for initiating haemodialysis process.
  • Responsible that all dialysis equipment is safe, complete and fully operational before, during and after each use. Include cleaning dialysis machine surface, work table and dialysis chair with appropriate disinfectant after each use.
  • Responsible to monitor and record patient’s vital signs during treatment and report any abnormality to staff f in charge.
  • Assist other staffs and doctors during treatment and examination of patients on treatment.
  • Perform other duties as assigned by Head Nurse.


Requirements:

  • Minimum 2-4 years of related experience
  • Diploma/ Degree in Nursing or equivalent
  • Registered with Malaysia Nursing board
  • Qualification in Post Basic Perioperative is as added advantage
  • Experience in renal care will be an added advantage.
  • Must be able to commit to rotating shifts.

JOB DESCRIPTION

The Staff Nurse is part of our Care Operations and Quality team that is experienced in nursing and carries out various types of nursing care, procedures, and methodologies in different settings, including home care services, with emphasis on the care recipient's needs.

In addition to home care, the nurse will also be responsible for attending to ad-hoc tasks, projects, and assignments as required by Homage, including but not limited to medical product usage, administration and maintenance of devices, patient education and counselling, public health education, and project-based medical assignments.

When not based in the hospital ward, the nurse must be flexible to perform care and nursing duties in other settings including home care, hospital, and facility-based care as assigned by Homage.

In this role, the Staff Nurse strives to ensure that the overall care recipient’s experience is handled seamlessly and delivered with high quality.

SKILLS & EXPERIENCE:

  • Minimum 1 year nursing experience in a General (Medical/Surgical) hospital ward setting or equivalent.
  • Must be a Registered Nurse/Medical Assistant with LJM/LPP.
  • Must have own transport.
  • Must be willing to travel to different locations around KL & Selangor.
  • Open to working during weekends & Public Holidays (rotation) and non-office hours.
  • Preferably with a Valid APC 2022 copy from LJM/LPP; otherwise, Homage may assist in applying for APC.

  • Prepares monthly reports according to Diaverum present standards concerning goals, achievements and problems of the clinic. Communicate with Country Nursing Manager on relevant matters.
  • Responsible for the clinic Infection Prevention and Control programs, identify and assigns Infection Control staff nurse in-charge.
  • Coordinate with Country Nursing Manager to ensure that audit action plans are completed.
  • Work as part of the Operations Management Team.
  • Collaborate with Operations on safety, cleanliness, and security standards for the clinic.
  • Consumption control and alignment with budget.
  • Regularly evaluates and monitors the use and replacement of equipment and supplies.
  • Coordinate supplies ordering and reviews stock control with stock in-charge.
  • Evaluates new products and leads training of new products.
  • Uses supplies and resources in a cost-effective manner.
  • Responsible for ensuring and monitoring water quality standard according to Country Regulations and Diaverum Water Policies.

  • Assigns patient care according to patient needs and staff capabilities and qualifications.
  • Ensures the utilization of quality assurance standards and protocols.
  • Assists with the development of clinic specific standards, and provides input.
  • Supervises and evaluates the quality of care through patient rounds and specific quality assurance projects.
  • Serve as a member of the Quality Management Committee and actively participate in the Quality Management Review meeting and follow-up.
  • Reviews, approves and analyzes incident reports.
  • Involvement in clinic audits, other quality certification processes and support in accreditation of clinics.
  • Acts as a point of liaison with medical and non-medical staff in coordinating multi-disciplinary activities.
  • Communicate and liaise with hospitals and other healthcare facilities.
  • Sets standards for and evaluates nursing documentation.

Job Description :

  • Provide support to plan, organize, and administer functions and activities in accordance with short- and long-term goals and objectives of the Hospital and the Professional Codes and Regulations for Nurses.
  • Work together with the Manager, Nursing Training & Development, and Director of Nursing in developing goals and objectives for the delivery of nursing care for nurses in Medical Centre that are consistent with the area of responsibility, mission, philosophy, and strategic plans of the organization and Nursing Philosophy.
  • Assist in the compilation of data for competency levels from all nursing areas in Medical Centre.

Clinical

  • Assist the Director of Nursing, to:
  • Review and enhance current policies and develop new ones to meet current needs. Conduct nursing training needs analysis.
  • Enhance department orientation program for new recruits and fresh graduates.
  • Implement a continuous evaluation system for the monitoring and assessment for staff to maintain their competencies.

Facilities Management

  • Recommend facilities and equipment that enables the nurses to deliver good nursing care.
  • Participate in the evaluation of specialized equipment and make appropriate recommendations.
  • Organize yearly training on specialized equipment by vendor.

Quality Assurance

  • Conduct internal quality audits with nurse managers on a regular basis as specified in KPI, to identify opportunities for improvement in the area of nursing care.
  • Assist in preparation of JCI and MSQH and assigned as Chapter Champion to perform audit, training, and relevant tasks accordingly.

Job Requirements :

  • Diploma in Nursing.
  • Post Basic Certificate in any discipline.
  • Able to communicate in English and Bahasa Malaysia.
  • State Registered Nurse with at least five (5) years clinical experience in the ward.

  1. Obtains report from the team she is relieving and record sufficient information to implement appropriate follow-up action as required.
  2. Provides report for the nursing team coming on duty, including sufficient information for follow-up action if necessary.
  3. Plans and executes Nursing Care for her patients with team members according to individual Patient’s needs.
  4. Administers and documents all medications, enteral nutrition, parenteral nutrition, and treatment as per Doctor’s orders and accurately record all care provided.
  5. Observes, evaluates, and documents all changes in the patients’ conditions. Notifies the Doctor, when necessary, of changes in the patient’s conditions, abnormal laboratory / x-ray results and any other unusual observations.
  6. Keeps Nurse Manager informed of all activities.
  7. Assists with the orientation of newly appointed Nursing staff.
  8. Provides ambulance escort duties for patient when required.
  9. Checks crash cart every shift to ensure all drugs and equipments are in place and in working condition.
  10. Carries out emergency resuscitation procedure in the absence of Medical Staff.
  11. Avails herself to all Emergency Response Plan (ERP) activities.
  12. Checks DDA and Psychotropic drugs every shift to ensure comprehensive documentation.
  13. Participates and contributes ideas at unit meeting and other committees toward improvement of the delivery of nursing care.
  14. Adheres to policies and procedures to meet PMC Mission.
  15. To participate in ESH activities.
  16. Able to identify safety hazards and immediate initiate corrective actions.
  17. Highlight any mechanical malfunction and activate corrective actions to be taken.
  18. Able to liaise with Ancillary and all supporting departments.
  19. Able to provide specialize care required for:
  • Unconscious patients, children, and babies
  • Isolation cases
  • Death Procedures

Education / Professional Qualification

  • The Job
  • SRN registered with Malaysian Nursing Board.
  • Must possess annual practicing license.
  • Preferably possess a post basic or Degree in Nursing.

  • Responsible for the clinic Infection Prevention and Control programs, identify and assigns Infection Control staff nurse in-charge.
  • Coordinate with Country Nursing Manager to ensure that audit action plans are completed.
  • Work as part of the Operations Management Team.
  • Collaborate with Operations on safety, cleanliness, and security standards for the clinic.
  • Consumption control and alignment with budget.
  • Regularly evaluates and monitors the use and replacement of equipment and supplies.
  • Coordinate supplies ordering and reviews stock control with stock in-charge.
  • Evaluates new products and leads training of new products.
  • Uses supplies and resources in a cost-effective manner.
  • Responsible for ensuring and monitoring water quality standard according to Country Regulations and Diaverum Water Policies.
  • Maintenance of water plant, monitors and all other medical- as well as other equipment.
  • Support clinic and country management.
  • Keep up to date with country regulations.
  • Support Corporate programs.
Management of Care

Coordinates and sets standards for the delivery of quality care.

  • Assigns patient care according to patient needs and staff capabilities and qualifications.
  • Ensures the utilization of quality assurance standards and protocols.
  • Assists with the development of clinic specific standards, and provides input.
  • Supervises and evaluates the quality of care through patient rounds and specific quality assurance projects.
  • Serve as a member of the Quality Management Committee and actively participate in the Quality Management Review meeting and follow-up.
  • Reviews, approves and analyzes incident reports.
  • Involvement in clinic audits, other quality certification processes and support in accreditation of clinics.
  • Acts as a point of liaison with medical and non-medical staff in coordinating multi-disciplinary activities.
  • Communicate and liaise with hospitals and other healthcare facilities.
  • Sets standards for and evaluates nursing documentation.
  • Participates in patient and family education.
Management and Communication

Interacts effectively with patients, family and staff to maintain standards of professional care to patients and families.

  • Promotes positive working relationships within the team.

  • Responsible for informing the Country Nursing Director and Human Resource (HR) Manager when employees complete the education programs and forwarding all required education documentation to the Country HR Manager.
REQUIREMENTS & QUALIFICATIONS

  • Diploma/ degree in Nursing or equivalent.
  • Registered with Singapore Nursing board.
  • Experience in renal care will be an added advantage.
  • Excellent English communication skills with the ability to handle inevitable objections, inspire trust and generate project enthusiasm.
