Description
  • Attend to incoming calls, retrieve, search, update system, and ensure Medical Records are provided in a timely manner.
  • Ensure Medical Records are not missing or misplaced; raise an “Incident” report if necessary.
  • Perform clerical duties in maintaining patients’ medical records, including filing, sorting, and processing.
  • Update HIS information and biodata, including merging records.
  • Compile data into medical records and ensure they are in order before filing at the designated storage area.
  • Understand colour coding and merge folders appropriately.
  • Maintain medical record functions through copying/scanning, filing, assembling, and retrieving patient records.
  • Follow up on the outstanding/movement of medical record folders.
  • Conduct thinning, archiving, and destruction processes to reduce inactive Medical Records.
  • Maintain strict confidentiality of any information encountered during your duties.
  • Enhance professional growth through reading relevant literature and attending training programmes (internal & external).
  • Perform other tasks/duties as assigned.
Requirements
  • SPM or Diploma in Healthcare Management or relevant field.
  • Minimum 2 years relevant experience in a tertiary hospital.
  • Good communication, data entry skills, and knowledge of PC, scanner, and printer operation.
  • Open to shift work (Morning, Afternoon & Night Shifts).

Medical Records Assistant

Reporting To: Medical Records Executive

Position Summary:

·    Perform Medical Records duties and other related tasks assigned by HOD.

Duties and Responsibilities:

1. Receive, compile, sort and check scanned and keyed in documents in EMRD from all the wards

* Check and receive all documents from the department/ward/clinic/unit.

* Sort and arrange all relevant documents accordingly.

* Scan documents as per document scan list.

2. Review medical records for completeness, assemble records into standard order, and file records in designated areas according to applicable filing system.

3. Follow-up on patient's out-standing MR, which is not dispatch on time.

4. Locate, signs out, and issue medical records requested by hospital departments.

5. Segregate and separate out files yearly based on year of last admission/visit.

6. Remove and shred all inactive files strictly in accordance to statute of limitations.

7. Enter and retrieve data and type correspondence and reports.

8. Print letter/reports and send to the respective doctors as per request.

9. Process medical reports

* Receive, check and record applications for medical reports with patients consent.

* Record on tracking system/logbook/white notice board and pass medical reports to the respective doctor's PCA.

* Follow-up/remind with PCA when the reports near to or meet the due date.

* Receive completed report from PCA, scan and process for collection as per requirement.

* Update record on tracking system/logbook/white notice board.

10. Assist the Medical Record Manager/Executive in carrying out Columbia Asia policies and procedures.

11. Attend relevant training and courses to meet needs of the department or hospital assigned by HOD or Management.

12. Participate in educational programs, in-service meetings and meetings as directed by the HOD or Management.

13. Provide individual consideration to the patients and relatives by ensuring their comfort and minimal waiting time for services.

14. Ensure patient records are accessible to authorized persons only.

15. Ensure "Security & Confidentiality" of patient records.

16. Collect and compile census or statistical data from other department/ward/clinic/unit, such as inpatient, outpatient, deaths, births, and types of treatment given.

17. Maintain Dispatch Listing/Records, MC records, and incoming and outgoing letter records.

18. Prepare, submit, maintain and safe keep the necessary monthly report as assign by HOD.

19. Perform other related duties as arranged or requested by authorized others.

Education and Requirements:

·    Minimum SPM/STPM qualification or equivalent.

·    Be familiar with medical coding and medical terminology.

·    Ability to work with people and work under pressure.

·    Ability to speak read and write in English language and Bahasa Malaysia.
